The unprecedented global mobilization during the COVID-19 pandemic revolutionized vaccine and immunotherapy research, reshaping how clinical trials are designed, executed, and evaluated. The rapid development and deployment of mRNA and viral vector vaccines demonstrated the potential of novel technologies to deliver safe and effective prophylactics within record timelines. Building on these advancements, the field is witnessing transformative innovations in antigen design, adjuvant development, and computational methods for optimizing immune responses. Concurrently, adaptive and decentralized trial models, empowered by digital platforms and real-world data integration, are redefining clinical evaluation standards. The knowledge and infrastructure gained from pandemic-era collaborations are now accelerating progress in immunotherapeutic approaches for cancer, infectious diseases, and autoimmune conditions. However, challenges remain in regulatory harmonization, long-term safety monitoring, equitable access, and public trust. This review explores how the lessons from COVID-19 continue to catalyze a more resilient, data-driven, and equitable landscape for vaccine and immunotherapy development.
